Three things, in our experience.

The structure is honest but undocumented. No plans exist. Framing is often full-dimension lumber, sometimes balloon-framed, occasionally sistered by a previous owner in a way nobody wrote down. We open small exploratory sections before we price structural work, so the proposal reflects the house rather than a guess.
